Records Request

To comply with Minnesota statutes and HIPAA regulations regarding patient records requests, Excelsior & Grand Chiropractic requires a signed and dated consent form from the patient or the patient's legally authorized representative. Once we receive your written request, we will process it within 30 calendar days as mandated by Minnesota Statutes section 144.292. Please note that this timeline starts when we resume normal business hours.

We request that you allow at least 48 hours for our response time. For any further information or to obtain the necessary forms, please fill out this form or contact our office directly.
